Endoscopic Treatment for Refractory Gastroesophageal Reflux Disease

Abstract
The incidence of gastroesophageal reflux disease (GERD) is increasing now in Korea. GERD symptoms are associated with a negative quality of life and increased healthcare costs and therefore require an effective management strategy. Although proton pump inhibitors (PPI) remain the primary treatment of GERD, they do not cure the disorder and can leave patients with persistent symptoms despite treatment. Moreover, it has been reported that there are many side effects of
chronic PPI use. Patients are still at risk of developing such complications as peptic strictures, Barrett¡¯s metaplasia, and esophageal cancer. Laparoscopic Nissen fundoplication has been the conventional alternate treatment for those patients who develop complications of GERD, have intractable symptoms, or wish to discontinue taking PPI. However, antireflux operations can lead to a new set of postoperative symptoms, such as dysphagia and bloating. The present report reviews the history of endoscopic treatments devised for the management of GERD and explores the outcomes associated
with the various endoscopic treatment modalities.
